Disk Unreadable

When i try to download a DLC for a game it seems to be working correctly until i actually play it.

When i start the game i got the DLC for it tells me the disk is "Unreadable".
I've tested this with fallout 3 and Mass Effect 1. Am i doing something wrong or is this a common issue?

1. Did you use modio to place the content onto your Device?
2. Did you leave the DLC on a USB Device?
